If your OSHA Outreach Trainer card has been lost, damaged, or needs to be reissued, you can request a replacement directly through the Trainer Portal.
Before You Begin
- You must have an active Trainer Portal account
- Your trainer authorization must be visible in your profile
Step-by-Step Instructions
- Log in to your Trainer Portal account
- Navigate to your Profile Page
- Locate the section listing your completed courses and trainer authorizations
- Find the course associated with your trainer card (e.g., OSHA 500 or OSHA 501)
- In the “Order Card” column, check the box next to the card you want to replace
- Click “Order Cards Now”
Important Confirmation Step
After clicking “Order Cards Now”:
- Select “OK” to confirm your request
- Select “Cancel” to stop the request
⚠️ Important:
- Ordering a replacement card will immediately assign a new card number
- This action cannot be undone
Additional Notes
- Replacement cards are only available for courses currently listed in your Trainer Portal profile
- If your course or trainer authorization does not appear, contact your OSHA Training Institute Education Center (OTIEC) for assistance
