Overview
If OSHA Outreach Training cards were mailed to an incorrect, outdated, or previous shipping address, the original cards cannot be redirected or recovered after they have been shipped.
To receive new cards, the authorized OSHA Outreach trainer must submit a Replacement Card Request through the OSHA Training Institute Education Center (OTIEC). Students cannot request replacement cards directly.
Before You Request Replacement Cards
Review the following information before submitting a replacement request:
Trainers may have multiple shipping addresses saved in the OSHA Outreach Training Portal.
Student course completion cards are mailed only to the shipping address entered when the course report is submitted.
Updating the shipping address in your trainer profile does not change the shipping address associated with a report that has already been submitted.
If tracking information shows the cards were delivered to the shipping address listed on the submitted report, the shipment cannot be redirected, intercepted, or recovered.
Eligibility Requirements
- Replacement cards may be requested only if all the following conditions are met:
- The student successfully completed the OSHA Outreach Training course.
- The original course report was submitted, and the card was issued.
- The course was completed within the previous five years.
- The replacement request complies with OSHA Outreach Training Program requirements.
- Common Reasons for Replacement Cards
- Replacement cards may be requested for reasons such as:
- Cards were mailed to an incorrect or previous address.
- Cards were lost after delivery.
- Cards were stolen.
- Cards were damaged.
- The original card contains a printing error.
- Replacement Card Request Process
- Verify the student's course completion and confirm that the original card was issued.
- Complete the OTIEC Replacement Card Request form.
- Provide the reason for the replacement request.
- Verify the shipping address before submitting the request.
- Submit the completed request to the OSHA Training Institute Education Center.
- Pay any applicable replacement card fee, if required by your OTIEC.
Once the request has been reviewed and approved, a replacement card will be ordered and mailed to the verified shipping address.
Important Information
Replacement cards must be requested by the authorized OSHA Outreach trainer. Requests submitted directly by students cannot be processed.
OSHA does not issue replacement Outreach Training cards directly.
Changes made to a trainer's profile after a course report has been submitted do not affect where the original cards were mailed.
To avoid delays, verify the shipping address carefully before submitting each course report.
